Abstract
Care planning is the third phase of the nursing process and the fifth stage of the clinical reasoning cycle (Levett-Jones & Hoffman 2013) (see Figure 14.1). Care planning can be defined as a systematic goal-based approach formulated in response to identified patient problems or needs. Therefore, planning patient care involves nurses in logically assessing (Chapter 13) and identifying a patient's problems (both actual and potential) or needs, setting goals/outcomes, establishing interventions (considering options, constraints and resources) and evaluating the results.
