Abstract

Healthcare service providers can take pride in great advances made in medical services delivery. Unfortunately, however, there is still room for improvement in preventing medication errors (MEs) that threaten the health of patients.1,2 Older patients perhaps might be most vulnerable to MEs. This is due to several factors including multi-morbidity, age-related changes in pharmacokinetics and pharmacodynamics, polypharmacy, fragmentation of care, and underuse of comprehensive older peoples' assessments.3
